What Is PTTD? ![]()
PTTD occurs when the posterior tibial tendon, which supports the arch of the foot, becomes inflamed or torn. This causes the arch to collapse over time, resulting in flatfoot, pain, and progressive instability.
It’s often caused by overuse, but factors like obesity, diabetes, or previous ankle injuries can also contribute.
Common Symptoms of PTTD:
-
Pain and swelling on the inside of the ankle
-
Flattening of the arch (especially on one foot)
-
Foot turning outward while walking
-
Difficulty standing on tiptoes
-
Increased fatigue in the foot and ankle after activity
What Happens If It’s Not Treated?
PTTD is a progressive condition. Left untreated, it can lead to severe flattening of the foot, arthritis, and permanent deformity that may require reconstructive surgery.
How We Treat PTTD at Third Coast Foot & Ankle
Every treatment plan is tailored to your stage of PTTD and your activity goals. Options may include:
-
Custom Orthotics or Ankle Braces – To support your arch and reduce strain on the tendon
-
MLS Laser Therapy – Reduces inflammation and encourages healing at the tendon level
-
Shockwave Therapy – Boosts blood flow and cellular repair in chronic tendon dysfunction
-
Immobilization (Boot/Brace) – Especially helpful in early stages to prevent progression
-
Physical Therapy – To strengthen surrounding muscles and improve gait mechanics
-
Surgical Consultation – For severe or late-stage PTTD not responding to conservative care
